If you're doing traditional UV texture painting, why not use something like Gimp? Unwrap and export UVs in Blender and then paint it in Gimp. The issue of texture size is more with how you're painting the texture and laying out the UVs, as I've seen some amazing work done in just 64x64 textures.

In 2.79, Blender Internal UI is showing an Add Texture Paint drop-down list under Slots panel.
